Based on your description, kindly follow these steps:
Choose the domain from your Authenticator app (the one in the image) > Settings (gear icon) > Remove account
If the account cannot be removed individually or the app should be completely signed out, clear all Authenticator data on the device: Open the device Settings app and select Apps > choose Authenticator from the app list > Storage or Storage & cache > tap Clear data > OK
If the account still appears afterward, please try to uninstall and reinstall the app.
