MS Authenticator MFA loop

Question

I am unable to sign in due to a Microsoft Authenticator authentication loop.

Behavior:

• I can enter my username and password successfully
• I receive the Microsoft Authenticator push notification on my device
• I approve the sign-in request in Authenticator
• The sign-in flow then returns to a prompt asking for a code
• Entering the code from Authenticator does not work
• The process loops back to the same verification step indefinitely
