Microsoft Authenticator App MFA Failure

I am using a personal account on my Microsoft Authenticator app.

I can successfully login to my Microsoft account (account.live.com) using an OTP successfully pushed to the Authenticator app on my Google Pixel 6 Pro. Additionally using the Authenticator generated 6 digit OTP works as well on my personal account.

However when attempting to login to the Azure Portal (portal.azure.com) I do not receive a push notification on my cell phone, despite the push notification being displayed on the website. Additionally, when attempting to enter a 6 digit OTP, a login failure occurs, indicating that "You didn't enter the expected verification code. Please try again."

Troubleshooting already completed:

  • Confirmed device time/date is set to automatic network sync (rules out TOTP clock drift)
  • Confirmed only one Authenticator entry exists for this account
  • Confirmed all notification and battery settings are correctly configured on the phone
  • Reviewed account.microsoft.com → Security → Manage how I sign in: all sign-in methods (email codes, text code to phone ending 3637, Authenticator app) show status "Up to date," and the Authenticator entry shows it was successfully used as recently as the day before this ticket was filed
  • Removed and re-added the Microsoft Authenticator entry using "Set up a different authenticator app" to force a standard 6-digit TOTP registration (rather than the default 8-digit passwordless-style code) — this new code is accepted by account.microsoft.com but is still rejected by Azure Portal with the same error

I’m the sole admin for my Azure/Microsoft Entra tenant and I’m locked out because Microsoft Authenticator is not working. I can’t sign in to Azure to recover access, and there are no alternate MFA methods available.

I have opened 4 incident tickets with Microsoft Help, and all have been "resolved" without the issue actually being fixed.

I spent an hour today arguing with a chat bot trying to get my issue re-routed to a team that can reset the MFA and restore admin access, without me having to login to the Azure Portal, which I cannot do. The chat bot just flat out refused, and eventually ended the conversation.

Suggestions Anyone??
