If the missing account is still sending approval prompts to the old phone, the QR code must be generated from that account’s security settings and then scanned on the new phone.

    For a personal Microsoft account:

    1. Sign in to the Microsoft account Security page.
    2. Select Manage how I sign in.
    3. Select Add a new way to sign in or verify.
    4. Choose Use an app.
    5. If Authenticator is already installed on the new phone, select Set up a different Authenticator app and continue until the QR code appears.
    6. Open Microsoft Authenticator on the new phone, tap the + icon, choose Personal account, and scan the QR code.
    7. After setup works on the new phone, remove the old phone entry from the account security settings.

For a work or school account:

    1. Go to the Security info page or the Additional security verification page.
    2. Remove the old phone or old Authenticator method if it is still listed.
    3. Select Add sign-in method.
    4. Choose Microsoft Authenticator or Phone, depending on what the account allows.
    5. Follow the prompts to show the QR code and scan it with Authenticator on the new phone.
    6. For restored work or school accounts, seeing “Sign in to add your account” is expected. Open that account in Authenticator and sign in again to complete setup.

If notifications are still going to the old phone, that usually means the old device is still the active Authenticator device. Approve the sign-in on the old phone if possible, then remove the old device from the account’s security settings.

If the old phone is unavailable and there is no other sign-in method such as SMS or email, the account cannot be re-added from the app alone. In that case, use another verification method if available, or have the organization’s admin reset or re-register MFA for that account.

If QR scanning fails, update Authenticator to the latest version first.
