This usually means either someone is trying to access the account, someone entered the wrong sign-in details by mistake, or a delayed prompt has arrived. For any prompt that was not initiated, select Deny every time.
Recommended actions:
- Check recent account activity
Review the Recent activity page to see sign-ins from the last 30 days, including device or app-specific information. If anything looks suspicious, change the password and update security settings. - Keep Microsoft Authenticator enabled
Continue using Authenticator and only approve requests that were initiated. If the sign-in screen does not show Authenticator first, select Other ways to sign in > Approve a request on my Microsoft Authenticator app. - If repeated attempts continue, use the sign-in helper
If the account appears blocked, locked, or continues to have sign-in issues after password changes, use the Microsoft account Sign-in helper. - If the account is a work or school account
After multiple attempts, Microsoft Entra multifactor authentication can temporarily prevent more sign-ins. Wait and try again later, use a different MFA method if available, and ask the IT administrator to review Microsoft Entra sign-in logs. If the phone is lost or stolen, the organization’s Help desk can clear MFA settings. - If Outlook mobile sign-in prompts are the source
In Outlook mobile, sign-in requests can be turned off from Settings > Accounts > account name > Account > Sign-in requests > turn off Allow sign-in requests.
If access to the account is still possible, checking recent activity and updating security settings are the most direct next steps.
