I would do it the following way:
- Install Windows Server 2016 on a "second" server
- Install SCSM 2016 on the "second server " as a second SCSM management server in the existing SCSM Management Group
- Install all the related software, for instance SMLets, ... on the new server
- Promote the second SCSM management server to be the primary SCSM management server
- Remove the "old SCSM server on Windows Server 2012R2" from SCSM Management Group
- Upgrade the "new SCSM 2016 on Windows Server 2016" with SCSM 2019
