A web-based tool in Microsoft 365 that enables users to quickly create surveys, quizzes, polls, and feedback forms.
Hi, Jaylyn Homnoan
Getting your account unlocked is a good first step, but Microsoft Forms has its own phishing check. That check can keep your forms blocked even after the account itself is restored.
Here are some suggestions you can try:
Give it a little time, then sign out and back in. When a Forms restriction is lifted, it can take 30 minutes or more to clear.
Open the blocked form for editing and look for a warning at the top. Remove anything that asks for passwords, login details, verification codes, payment or banking info, or too many personal details. Taking out the flagged wording can lift the block.
Make a copy of the form (click ... > Make a copy), double-check the wording, then share the new link. The copy gets a new form ID, so it often works when the original stays stuck.
If this is a work or school account, ask your IT admin to review it. They can unblock both the form and any Forms restriction on your account.
If it's a personal account (Outlook.com, Hotmail, etc.), there's no self-service way to unblock it. Go to https://support.microsoft.com/home/contact, sign in, and ask for a false-positive review. Keep the original form while you wait, and don't delete it.
Please be advised that this is a user-to-user support forum. Moderators and contributors, including external Microsoft employees, are not allowed to directly interfere with Microsoft product features or access back-end systems. Our role is limited solely to providing technical guidance on reported incidents, requests, or ideas.
Also, can you let me know if you're using a personal or a work/school account? Please share the exact message you see too, but leave out any personal details.
