What's the difference between the category of "Allow Required" and "Default Required"?

All of these need to be allowed through all your network infrastructure if you want Microsoft 365 services to function as expected.

As michev told you in his reply, to guarantee Office 365 services works as expected, all the IP address and URLs should be allowed.
Even though I only use Microsoft Teams, shall I also keep the address related to "skype"?

It recommneds you to add all the addresses.
Skype for Business and Microsoft Teams have many associations. So, it's hard to guarantee of Microsoft Teams functions if excluding the Skype-related address.
