1.Please add the actions "log to history" before and after action "send mail" to check if the send mail action is performed.
2.Please check if the alert email works well when you create a list/library alert to User A.
3.Use Telnet to test SMTP and Exchange outside of SharePoint, If this works, the issue is on the SharePoint side, or between SharePoint and the SMTP relay, if not, you likely have an Exchange problem.
- Install “Telnet Client” (as a feature through Server Manager) on the SMTP Server.
- Open Command prompt and type in the following. Note: You will need to change "dc.contoso.com " to your Exchange server, and the two email addresses to ones appropriate for the customer.
