Azure b2c- Error in signup or change password ocassionally

We are dealing with a problem where while signup or change password process we are unable to signup a new user. It displays an error- The server has encountered an internal error. Please try again later
and sometimes it gave
cannot verify the codes, please try again later

and when going through audit logs it seems there is a slight frequent failure
Activity Type- Issue a SAML assertion to the application
Status-failure
Status reason- There is a problem with the service.

It is happening occasionally. Sometimes it works fine. What could be the issue?
We are using custom policy. Any leads so that we can figure out what is the actual problem.
