I have one upstream and one downstream on server 2022
when upstream synchronized completely through internet downstream shows them after synchronization but doesn't download them
in options download approved updates are not marked and I don't have event id 364 and permissions are OK
downstream hasn't internet connectivity
in wireshark on downstream shows it wants to connect to slscr.update.microsoft.com
when I established internet connectivity problem was solved!!! after that I disconnected it and downloads new updates in upstream, downstream synced successfully without any problem and doesn't need Internet connection!!!
I tried new downstream and again it doesn't download any updates, I imported updates from upstream and then it shows all of them are downloaded, then I downloaded new updates in upstream and when synced with downstream and again downloaded 0.00 MB. then if I don't want downstream hasn't internet connection at all I should import updates every time!!!

Please help to confirm the downstream WSUS is in the Autonomous mode or in the Replica mode. In my opinion, the downstream WSUS in Autonomous mode do not receive update approval status or computer group information from the upstream server. The clients who poing to the downstream WSUS will approve the updates individally and download the updates from the Internet.
The Replica mode, also called centralized administration, works by having an upstream WSUS server that shares updates, approval status, and computer groups with downstream servers. Replica servers inherit update approvals and are not administered separately from the upstream WSUS server.

Hi, it's autonomous mode. I said about approvement in downstream to explain it doesn't need approve for download new updates
In my opinion, we have to approve the updates in the autonomous downstream WSUS server. Perhaps we could change the downstream WSUS into the Replica mode. Replica servers inherit update approvals and are not administered separately from the upstream WSUS server.
I said I unchecked download when approved in downstream, because upstream is not joined in domain and I don't need manage centrally I use Autonomous mode. my problem is why downstream doesn't download updates and I should connect it to internet!!!
As far as I know, the upstream WSUS must have all the metadata and Binary update file before you try to approve the updates in the downstream WSUS server. The upstream WSUS is set up as the source to which the downstream servers synchronize.
Please help to confirm whether all the approved updates in the downstream WSUS are downloaded on the upstream WSUS first.

my problem is not about approvement. when upstream synchronized completely, I sync downstream it says you have 100 updates 1000 MB for example and 0.00 downloaded!!!
when I connect downstream to internet for seconds first time it cam download all updates from upstream and after that I don't need connect it again. I ask why???
I saw previously bits have problem if you don't have default gateway in installing SCCM clients, and in my lab for trying downstream I didn't configure it. now I add fake pingable default gateway and problem is solved!!!

Run a WsusUtil reset on the upstream
& "$env:ProgramFiles\Update Services\Tools\WsusUtil.exe" Reset
Wait 24 hours
See if any improvement to downloading happens.
I should deploy another downstream and try your solution
