Hi,
I'm having trouble understanding what Last Exchange Activity inside the services usage report means. I have accounts that report exchange last activity as being way back in 2020, but a message trace on the account turns up emails in the last 90 days. I'm at a loss as to understand what exchange last activity means, does it not include emails sent/received and only including logging into exchange or other activities within exchange.
Please advise.

Last activity date refers to the last time the user performed a read or send email activity.
No receive events are counted, only send ones.
Well then something is going on, as the message trace shows the user as sending 2 emails within the last 90 days and the last activity date on the report is in 2020. Any thoughts as to why the discrepancy?
I believe you are looking at email activity, the one in question is last "exchange" activity.
It might just be a delay in updating the report's data, that's not uncommon. The data for "last exchange activity" should match the "email activity", so it shouldn't matter which one you are looking at.
