approval flow not working correctly with multiple approvals

Question

Hey everyone,
I have a form that is submitted and needs to be approve/reject from 2-3 individuals. The flow runs perfect if only 1 person has to approve/reject it, but the moment I add in the second or third person. Everything triggers the rejected notification even if all users approve the form. Any Assistance would be appreciated as I am new to Power Automate. I am using the start and wait offer approval option -
