Hello,
Is anyone having issues with MAC clients adding Exchange 2019 on-premises mailboxes? I can't add them anymore to MAC clients using native MAC Mail or Outlook for MAC in classic mode. We are running in full hybrid mode, but all our mailboxes are still on-premises. iOS devices are still working fine.

Could you please provide any screenshot about the issue you encounter failed adding client to your server, do you mean you failed to configure the profile? What detailed error did you receive?
And when does this issue occur, did you make any changes to your server recently? Do other outlook clients have this same issue?
Make sure your client version have met the requirement: Supported clients (with latest updates) in Exchange 2019
And you could also check if your issue is related to this: Outlook for Mac clients cannot connect to Exchange Server
