Turn on "iCloud Drive" backup under Settings -> iCloud (first button that is probably your name) -> iCloud -> iCloud Drive.
It looks like the backup of the Authenticator app data falls under this category so if that section is off then it can't save the account info.
Microsoft Authenticator: "backup is out of date" Apple iPhone
Every time I open my authenticator app on my iPhone I get this message:
"Your backup is out of date. Sign in to your iCloud account and go to Settings to update your backup."
However, authenticator is set to backup in iCloud settings.
I will have to change my iPhone soon and I'm worried I will lose everything here and start from scratch. How can I fix this?

2022-10-19T16:03:24.66+00:00 I was having the same issue and found that my issue was resolved after navigating to Settings -> (the first button that is probably your name) -> iCloud. Then under "Apps Using iCloud" I selected "Show All" and found that Authenticator was listed in that list and set to off. I turned it on, waited a few seconds and opened Authenticator which no longer prompted me, though it did have a red dot on the "hamburger" menu in the upper left. I went there and it advised me about which account my Authenticator codes would be backed up to and hasn't popped since.
Note that I did all of this after making the change that @Keith Rackleff suggested, so not sure if that was a factor.

2023-01-20T10:40:59.3733333+00:00 I just had this issues and it was because I had to agree to some new "terms and conditions" from Apple. Open settings and see if there is a new "Terms and Conditions" prompt. If so, open it and "Agree".
