How to add a ColumnHistory Field on access form

I want to be able to see the note history on a form i created.
I have the notes field and i added a 2nd field for the notes history - called NotesColumnHistory
What would the formula be to add this function to my form?

Table Name: EcgsAdvisorList
Notes field - is set to Append Only "Yes"

Please help!
