Email notification when create or delete user account

Hi All,

Ceated alert rule to trigger email notification when the user is created, update and delete.
However, when users created, update, and delete, I received the email notification but the field information is not the same as the query result. E.g no initiated by field etc on email notification

Please kindly assist.
