IIS Manager: IP Address and Domain Restrictions not working

In IIS Manager we have IP restrictions set on one folder of our web. We are noticing that some IPs are gaining access even though that IP is not listed among the "Allow" mode in IP Address and Domain Restrictions. In what instances would that happen? Thanks

    Restrictions have been set inside IIS Manager>Security>IP Address and Domain Restrictions

    What config info do you need? For all IPs that we allow, we have added an "Allow Entry" for each. No "Deny Entry" has been set. We have tested numerous anonymous access attempts for various IPs and all works as expected. We just finding it weird that an odd IP every no and then is reported as having been allowed access without that IP having explicitly been added as an allow entry

    What config info do you need?

    The configuration information of this part of the <security> node and make sure the website you set is the website you are testing with.

    Also note that once denied IP addresses have been added, click Edit Feature Settings... and select Allow for Denyfor unspecified clients. Click OK.
