Validate |
Verify that the questionnaire has been set up correctly. |
Result groups |
Manage the result groups that are used to calculate points and categorize the results of a questionnaire. For more information, see About result groups. |
User rights |
Specify the questionnaire groups that a questionnaire is available to. For more information, see About questionnaire groups. Note There are additional ways that questionnaires can be made available to respondents. For more information, see Distributing questionnaires. |
Questions |
Maintain questions on the questionnaire. |
Conditional question |
Define a conditional question hierarchy to make the order that questions are presented dependent on the answers that are selected by a respondent for each question. For more information, see About conditional question hierarchies. |
Edit |
Add or change a note that describes the selected questionnaire. |
Fields

Description |

Active |
Select this check box to make the questionnaire active. An active questionnaire is available to all employees if user rights are not specified. Note There are several ways to make a questionnaire available to respondents. For more information, see Distributing questionnaires. |
Questionnaire |
Enter an identifier for the questionnaire. |
Description |
Enter a unique description of the questionnaire. Questionnaire descriptions are displayed in the list of questionnaires that can be selected by respondents. |
Type of questionnaire |
Select the type of questionnaire to create. For more information, see About questionnaire types. |
Total time |
Enter a time limit by when respondents must complete all mandatory questions after beginning the answer session. The time limit should be entered in in hours, minutes, and seconds (hh:mm:ss). The total time cannot exceed 24 hours. During the answer session, a countdown is displayed in the upper right of the form next to the Time remaining field. When time expires, the answer session ends, and no additional responses can be provided. If respondents do not complete all mandatory questions, no results are recorded for the answer session. |
Form template |
Select a form template, which is a graphical background that is shown during an answer session, from the list. For more information, see About form templates for questionnaire. |
Save question |
Select this check box to:

Sequence |
Select a sequence for the questions from the following options:

Percent |
If you selected Random (percentage in questionnaire) in the Sequence field, this is where you define the percentage of the questions that are assigned to the questionnaire to present. Note If you selected Random (percentage in result groups) in the Sequence field, you will define this value in the Result groups form. |
All |
Specify whether all questions must be answered before a questionnaire can be approved. |
Back |
Select this check box to allow respondents to return to an earlier question during an answer session. |
Overview |
Select this check box to activate the Overview button and allow respondents to see an overview of all questions on a questionnaire during an answer session. |
Note |
Select this check box to activate the Note button and allow respondents to add more information about their answers. |
Point statistics |
Select this check box to activate the Result groups button and allow points to be calculated for the questionnaire. For more information, see About result groups. |
Answer report |
Select this check box to allow an Answer report, which provides an overview of the questions asked and the answers selected, to be printed for the questionnaire. This option also enables an Incorrect answers report to be generated. |
Results page |
Select this check box to be able to print a results page when the questionnaire has been completed. |
Evaluation |
Select whether the answers should be evaluated. Depending on your selection, you must select one of the following options:

Origin |
If you selected Questionnaire in the Evaluation field, specify how to evaluate answers from the following options:

Value |
Specify the value to be achieved to pass the questionnaire. Depending on your selection in the Origin field, you must indicate:

Graphical indicator |
Select the graphical indicator to use to show questionnaire results. |
Note |
Add or modify notes for the questionnaire. |
