Date

Specifies the date of the report.

Place

Specifies the location that should be printed on the report.

Name

Specifies the name that should be printed on the report.

Designation

Specifies the designation of the person responsible for the report.

Duplicate

Select to print a duplicate of the report.

Son/Daughter of

Specifies the relationship of the person working as the assessee.
