When to use

Use a brief clinical monologue when the patient is sleeping, turnover moves quickly, or you finish a task after leaving the room. State the same discrete documentation details you would include in a patient-facing conversation.

Apply the same verbalization habits you use during patient conversations. State:

  • The care activity or finding.

  • The location and laterality, when relevant.

  • Specific quantities and units.

  • The patient’s response or the outcome of care.

Scenario Verbalization example
Out of bed to chair "Out of bed to chair with one assist, able to stand and pivot, tolerated well."
Ambulation in the hallway "Ambulated in hallway with use of wheeled walker, standby assistance. Nonskid footwear on. Gait is steady. Ambulated for a total of 75 feet. Tolerated well."
Repositioning in bed "Repositioned in bed to the left lying side with two staff assist. Heel protectors on. Bed in low position. Bed alarm on. The side rails up two times. Call bell in reach."
Trust and rapport "Plan of care explained and questions answered."
Emotional state "Patient calm and cooperative." Or: "Patient sad and verbalized being depressed. Asked to speak with someone. Chaplain services notified."
Incontinence and skin care "Incontinent of urine. Peri-care provided; perineum cleansed. Gown, bed pad, and linens changed. Coccyx reddened. Skin moisture barrier cream applied."
Urine output "Urine output 350 mL from Foley. The urine color is yellow and cloudy."
Intake and output "Intake of 350 mL. 75% of meal consumed. Drank 50% of Ensure. 300 mL of urine output, clear and yellow in color. Patient had one unmeasured bowel movement."
Pain assessment "Complains of lower back pain, radiating to left leg. On a 0-10 pain scale, he rates it as 6. Described as sharp, shooting pain. The patient stated pain goal of 3. Medication administered."
Vital signs "Temperature is 100.5, taken orally. Heart rate of 98. Respirations 18. Blood pressure is 100/50 in left arm. O2 sat 98% on 2 liters of oxygen via nasal cannula."

Verbalize daily care activities

State each daily care activity as you complete it. It helps Dragon Copilot draft the appropriate flowsheet rows and reduces the need to reconstruct details from memory at the end of the shift.

Include details such as:

  • Positioning: 'Head of the bed elevated.'

  • Support devices: 'Pillows and positional supports in use.'

  • Incontinence care: 'Incontinence pad changed.'

  • Hygiene: 'Teeth brushed. Perineal care performed. Patient bathed with prepackaged wipes. Gown and linens changed.'
