How can I configure a proxy server with a username and password on a Windows operating system?

How to configure proxy with username and password in windows. Also, how can I prevent repeated prompts for proxy authentication in web browsers and various applications? Is there a way to automatically handle these credentials to avoid these repeated prompts?
