Receiptent Email Address Changes AFTER Email is Sent - ONLY in OWA

We have one person in our organization that is having this problem. When she sends the email to a specific email address, she enters the address manually, it shows correctly in the To field and she hits send. After it's sent, she looks in her Sent folder and the address is changed to the recipient's old email address.

This issue is ONLY occurring in OWA, and can confirm it happens in multiple browsers. The issue does NOT occur in the Outlook client, only OWA.

In the Outlook client we had her click the "Empty Auto-Complete List" because we couldn't find that option in OWA. Had her log out of OWA and log back in and the issue persists.

We also had her clear her browser cache and checked if there were any entries in her personal address book, and there's not.
