Outlook 2016 Forms are blank when received

Created a form to be used by my company but when I have someone test the form it appears in my mailbox completely blank. If I select the "forward" option the populated fields appear but when you hit Send the receiver sees nothing but a blank email

We are running Exchange 2016 on a on-prem server in a domain environment.

    Where did you create this form? Via Outlook or other programs? According to my tests, I created a form in Outlook when editing emails and sent it to others users in HTML format. And other users can see the content in the forms normally. To confirm if the issue is related to this specific form, please try to do tests on other forms and see if this issue could also be reproduced.

    Which email message format did you use when editing these emails? HTML or Rich Text or Plain Text? Please try to send emails with other format and check if you can normally see the form contents.

    Besides, can you see the emails with the specific form on your web mail? In order to confirm if your issue is related to your Outlook desktop client, it is suggested to send emails with forms on your web mail and check these emails on the web mail as well to see if the issue has any difference.

    I have the same problem. The form was not published but the handful of people that need to use it, all have it setup locally and we testing between those individuals. I've tried changing to Plain Text and Rich Text but it doesn't change anything. It's simply stripping anything you entered into the form when you send it.

    I've seen a few other sites with people asking for help with this same issue but so far, no solutions have been given.

    Same issue here. Form is submitted and when received all fields are blank when opened in Outlook. I know the data is there as I can save the e-mail as an HTML file and I see it when opening that. Only using text boxes and a couple of checkboxes and radio buttons in this. Any ideas. Thinking it's gotta be a view setting in Outlook somewhere. Any feedback would be much appreciated.
