Hi PamelaMahoney, to better understand and solve your issue, we may need you to provide us with more information related to your issue:
Created a form to be used by my company but when I have someone test the form it appears in my mailbox completely blank.
Where did you create this form? Via Outlook or other programs? According to my tests, I created a form in Outlook when editing emails and sent it to others users in HTML format. And other users can see the content in the forms normally. To confirm if the issue is related to this specific form, please try to do tests on other forms and see if this issue could also be reproduced.
Which email message format did you use when editing these emails? HTML or Rich Text or Plain Text? Please try to send emails with other format and check if you can normally see the form contents.
Besides, can you see the emails with the specific form on your web mail? In order to confirm if your issue is related to your Outlook desktop client, it is suggested to send emails with forms on your web mail and check these emails on the web mail as well to see if the issue has any difference.
