The following table provides descriptions for the controls in this form.

Fields

Field	Description
Type of rate	Enter the identification code for the rate type. Bemærk A rate type that has the Over-rate check box selected is used only to indicate the wearing period.
Description	Enter a description for the rate type.
Over-rate	Select this check box to indicate that the control by rate is required.
