Page of Radio Book

The screenshot is the top part of a page from a Dutch PDF on Electronics Theory I am trying to reformat into English - the original documents were scanned to PDF as images so I've had to convert them! What I am trying to do is to make the Header Title "Radio" and Test Boxes for the Part, Chapter and Page Numbers automatically format on opposite ends of a line that prints on each page in the header (I think its called an "Accent Line" - how do I do this please, and is the a ready-made template like this?
