Getting an error on the audio iotedge module. Middle LED just flashes white. " IO reported an error " is seen in the logs (as seen below)

Hello @Carey Payette , can you confirm that you followed the troubleshooting steps from Troubleshoot Azure Percept Audio and speech module doc? See as well Azure Percept Audio button and LED states. What steps you tried already? I would request you make sure all updates are there.

If the center LED is always blinking, it indicates an authentication issue. Try these troubleshooting steps:
1- Make sure that your USB-A and micro USB connections are secured
2- Check to see if the speech module is running
3- Restart the device
4- Collect logs and attach them to a support request
5- Check to see if your dev kit is running the latest software and apply an update if available.

Yes, I have been through the troubleshooting. The azureearspeechclientmodule fails to start (the log is attached to the original post).
I have updated the device using OTA updates (including hard-stop release up to 2021.109.129.108), and restarted the entire procedure by updating by USB (I only did this one to version 2021.107.129.116 as a test). The module was not able to start in both instances. The log shows the authentication is successful ([m [1;32m21:20:13 Info: connection status callback: status=<IOTHUB_CLIENT_CONNECTION_AUTHENTICATED>, reason=<IOTHUB_CLIENT_CONNECTION_OK>), yet the board continues to blink.
I am unable to attempt deployment of any of the demo models as percept studio thinks the audio device is disconnected.
I can try USB updating to the latest version, but I have a feeling my audio hardware may be faulty.
Hello @Carey Payette have you tried the USB updating? I may need to look at the logs collected using support-bundle. If you have a support plan can you please Create an Azure support request?

Thank you for following up - I haven't done the USB update to the latest version yet (will do so over the next couple of days). I did try the USB update to version 2021.107.129.116 and the issue persists.
I will update you here when I try the latest version.
Updating via USB to version 2021.104.110.103 fixed the issue with the edge audio modules :)
