    Hello Nicholas,

    Welcome to the Microsoft Community.

Understanding that you are unable to access a form created on 6/2024 the issue prompts that the form does not exist and multiple computers are experiencing the same issue. I understand the inconvenience this causes.

In order to better understand the full scope of the issue, I would like to know the following information:

    1. Is everything ok with the form creator's account and also not accessing this form properly? Is the newly created form accessed properly?
    2. There are different troubleshooting options for different types of accounts, you seem to be located in a corporate environment, what type of accounts do you use, work or personal?
    3. Sometimes this can also be caused by temporary fluctuations in the Forms service, wait a few hours or a day to see if it is still inaccessible.

This information is important for us to continue to help you and we look forward to hearing from you.

    Best regards,

Ian - MSFT | Microsoft Community Support Specialist

    Hi Nicholas,

Thanks for your updated sharing and I understand you have tried and spent a lot of time on collecting the forms owner information so that it can help you and your team to continue the work. In this situation, my recommendation is to contact the Microsoft escalated online support team where the backend support team has more permission than us, they can offer to help with more resources to investigate the form owner ID from JSON for you and locate from Azure.

I understand how important it is to get this issue resolved. In this situation, we sincerely recommend that your tenant Microsoft 365 admin open/create a free M365 support ticket with a Microsoft support engineer. This would be the most efficient way to handle this case for you and your team regarding to this form issue. Reference: Get support - Microsoft 365 admin | Microsoft Learn

Note: To create a support ticket, you must have Microsoft 365 admin privileges, and if you're not an admin for your Office 365 organization, contact the admin for creating a new ticket. Reference: How do I find my Microsoft 365 administrator? - Microsoft Support

Please accept my apologies and regret that I did not resolve the issue and redirected you to the related development team, but please understand that I'm redirecting to the right support channel to better address the problem. That's why we suggest you contact your administrator or your IT department. They can create a support ticket. Once your administrator or IT contacts them, they will contact you back within 30mins to start the remote session.

I am sorry and apologize for it.

Your understanding and patience will be greatly appreciated. I hope you stay safe and healthy!

    Sincerely,

    Mia

I do not know who the Creator of this form was and I suspect a manager or some form of controlling department that would generate this style of communication. Since then and after a few questions to my supervisor I believe this person has either been assigned to another task or possible not with the company and their account was altered or removed which took this form offline. Since then a new form was created is my suspicion and we are back up in running once again.

I have tried to teach myself and learn new things by utilizing the above response to see if I can find the creator/owner on both the old and new form and not having a lot of success. The old one I can navigate the information an get a lot of line items but unable to narrow down in the Json to find an OwnerID or really any matching content shown in the example instructions ( Step 1: Find the Owner ID from Web Browser Tools). Maybe because the page does not exist that all the important data is gone. As far as the 2nd part for looking up the OwnerID if I had an ID I do have access to that to look up information,

    Dear respected Nicholas,

I'm Mia and following up your thread regarding to the form error scenario. I went through your post and updated information and since I noticed you mentioned this same error can be reproduced across various browsers and devices, I agree with your feeling that there might have some owner account issue that caused the form got disappeared like the original storage location with that form is expired or non-existing.

Hence, to find out who is the creator/owner of the issue form, even though currently there has no out-of-box function to achieve it, I did manage to find a related thread where some other users have confirmed the workaround can indeed find the form owner. (Note: From my check of the article, it may require the admin account permission to accomplish the process for locating the owner account in Azure)

After that, confirm with the admin to check if the owner account service is in health from Microsoft Admin Center>Users section, especially its OneDrive service as it's the place where personal created forms stored. In case you need: How do I find my Microsoft 365 admin? - Microsoft Support

Please correct me if I made any misunderstanding and I will be here waiting for you to offer any further support.

Appreciate your time and patience. Hope you a nice day and stay safe!

    Best Regards,

    Mia

thanks for the reply and here is what I know below per your questions.

    1. I do not know who the creator is for this form and I am wondering if this person may not be with the company and it went down after that. I have ask my superiors why the form when down and who might be working on it to fix it.
    2. This is a work account and work form that was created to ask specific questions for making request and then sends an email to correct individual that can help.
    3. This form has been down for 5 working days now. Started on 2/26/2024.

To me I might be asking who the creator was and see if that leads to the cause and effect.
