Even if the form is gone, the "instruction" to require that data remains in the library settings, which is why Teams is flagging every file as "incomplete." Because Teams is essentially a window into SharePoint, we need to go to the source to delete that property.

Since Teams is backed by SharePoint for file management, you might want to check your SharePoint document library settings. Navigate to the document library where your files are stored and see if you can manage the columns from there. Look for library settings under "Column" and see if the "Song Requests" column can be deleted.
Step 1: Open the folder in SharePoint
Teams doesn't allow you to delete column properties directly from its interface, so you'll need to jump to the backend.
- Navigate to the Files tab in your Team where you see the error.
- In the top menu bar, click the three dots and select Open in SharePoint. (A browser window will open).
Step 2: Access Library Settings
- Once in SharePoint, click the settings (gear icon) in the top right corner.
- Select Library settings, then click on More library settings.
Step 3: Remove the "Song Requests" Column
- Scroll down to the Columns section.
- Locate and click on the name of the column: Song Requests.
- Scroll to the bottom of the column edit page and click Delete. Note: If you want to keep the column but stop the error message, you can instead change "Require that this column contains information" to No.
- Confirm the deletion when the pop-up appears.
Step 4: Clear Cache and Refresh
Sometimes a simple refresh or clearing your Teams cache can help resolve interface issues.
When you tried creating the form through certain Teams integrations, it likely triggered a "Metadata" requirement. In SharePoint, if a column is marked as Required, any file uploaded even a simple notebook, or document will show a warning until that specific field is filled out. Deleting the column removes the requirement entirely.
