chapters 1-16, then chapters 17-23 sill display the Chapter 16 title. From chapter 24 on, it works properly again
For the chapter titles in Chapters 17-23 verify that you are using the style sought by the StyleRef field on the entire paragraph.
Click anywhere in the paragraph used for the chapter title (without selecting anything) and re-apply the style.
When a linked style is used on part of a paragraph, perhaps even all of the text but not the final invisible paragraph mark, the character formatting for the style is applied to the selected text, and it will show up as having the style applied, but the StyleRef field will not see it. When you click in a paragraph without selecting any text, a linked style will apply the paragraph formatting to the entire paragraph; when you apply it to selected text, it only applies the character formatting to the selected text.
In the screenshot we see text that has the Heading 1 style applied, although not to the entire paragraph. The StyleRef field does not see it, though. If the style is applied to the entire paragraph, it will see it.

I always keep the option to disable linked styles checked.
