This usually means Word has switched to a different speech route after an update, so the natural voice is still available somewhere on the PC, just not being picked up properly by the desktop app. Word and Windows/Edge do not always use exactly the same voice source, which is why a voice can still exist in one place and disappear in another.

Open the same file in Word for the web, then go to View > Immersive Reader and test the voice there first. If the natural voice works there, it points more to a desktop Word profile/settings problem than the voice being gone completely.
It is also recommended that you test with a brand new blank document, and if that works differently, rename Normal.dotm so Word rebuilds its user template/settings. It is also suggested that trying a different Windows user profile can help confirm whether the issue is tied to your current profile only.
If you need a workaround right now, it is suggested that you save the document as PDF and use Microsoft Edge Read Aloud, or use Word for the web until the desktop app starts pulling the proper voice again.
