Just to clarify - has the deactivated account been inactive for more than 30 days? This is important because Microsoft Forms data is directly tied to the user account that created it. Once an account is deleted and passes the 30-day retention period, both the account and its associated Forms (including responses) are typically permanently removed and cannot be recovered.
If the account is still within 30 days, your Microsoft 365 admin may be able to restore it and access the Forms. You might also try the Microsoft Forms delegation page to recover the form:
- Go to: [https://forms.office.com/Pages/delegatepage.aspx?originalowner=[owner.email.address]
- If the account is still within the 30-day grace period, you should be able to transfer ownership of the form to yourself or a group.
If the account was deleted over 30 days ago, Microsoft permanently removes all associated Forms data, including the form, any responses, and access via shared links.
