I had the same problem which started the other day. This morning I was poking around the settings and noticed that when I go to Settings of the Authenticator app, under Device Registration it was registered under a company I worked for previously. I clicked to deregister and somehow the authentication from Outlook and Teams is now working. Not sure if this was causing the problem or making this change reset something, but worth checking yourself.
Issue with Microsoft Authenticator on iPhone 11 - iOS 16.0.2
It seems that Microsoft Authenticator crashes when tries to send the response back to Outlook/Teams.
This happened suddenly yesterday.
I tried deleting the app and downloading again, with fresh setup of accounts within, with no luck.
Can you please assist ?

Are you on Auth App 6.6.5? Just check the version is also updated from App Store, the other option is to request a reset of the MFA from the console and you should be able to add the accounts for retest. howto-mfa-userdevicesettings

Perhaps some issue with 6.6.6 version, considering that it is the Authenticator App that is crashing and does not send replies back to the applications (Outlook, Teams). -
