I believe this issue is a result of mt denying an access request. I had access and no issues with my work account (outlook and teams) on my phone through authenticator. Then a few weeks ago I got an alert to approve an access request that was from a different city than I am in. So I denied it. I wonder if it was someone from our IT department or myself that was the access that I denied, and it just showed the wrong location.
How do I get my account back in authenticaor? It is still listed there, but is grayed out and I can't click on it. I have tried re adding the account, but it tells me that I have already used that QR code and the manual url.
I don't know how to get my work account set up to work again.

Maybe your IT admins can help you revoke the authentication and re-register your authenticator.
I do not have a Passwords/Accounts section in settings.
Ok, well that was an example for an android device. Your mileage may vary. What device are you using?

No. My IT team has reset/unregistered my account and I have tried adding it again, and it is still giving me the error that this QR code has already been used.

Awful. Exactly the same situation. But the account is grayed from the day I unpacked my working laptop.
Admin is not a gennie and has the same questions.
All we do does not help. The authentication requests do not come to my mobile, and the account is grey when seen from the mobile. The mobile itself was registered in the company manually, and "activated" by the admin later.
The MS Azure support promises to contact me later, and that "later" is already 9 months old.
A baby may be born, but MS is still procrastinating somewhere.
This Authenticator thing is pure sabotage and should be banned everywhere and forever as an unconventional weapon

I had the same issue, but I logged into my Microsoft using my LiveID credentials and it prompted me to add authenticator. I followed the procedure and the account went from grayed out back to good. Support was really not a great help with this black magic.
When I log in to the MS account (it is not Live iD, because it is a corporate account in Azure), it requires me to authenticate on a device which initially was not responding and now completely removed; it still sends the request to the void, and waits me to respond there.
