I have tried everything but for some reason my windows defender is broken and I cannot find a fix short of a refresh of the OS

It says "Your PC couldn't be scanned. Some history items could not be displayed. Please wait a few minutes then try again. If that doesn't work, clear the history and then try again."

under my history there is nothing ..

As Mr. Boots said, it may be a disk error.  Alternately, some files Defender requires may be corrupted.

  1. Disable Windows Defender in Settings tab under Administrator section
  2. Delete the folder C:\ProgramData\Microsoft\Windows Defender\Scans

It may be hidden, and will require elevation to access it.

  1. Enable Windows Defender using the Security Center notification on your task bar

When you enable Defender, the files should be recreated.

    Did Windows 8.x come with the PC or is this an upgrade?

    The error means:  ERROR_MP_BAD_GLOBAL_STORAGE 

    Have you checked the disk for errors?

    Try replacing the signatures by downloading the full engine/definitions from the portal and applying them: https://www.microsoft.com/security/portal/default.aspx
