I'm sorry to hear that you are experiencing issues with attachments missing from approvals submitted through the Approvals app within the Teams client. This issue has been reported by some users and Microsoft is currently investigating the root cause of the problem.
In the meantime, you can try a few workarounds to see if they help resolve the issue.
- Try using the Approvals app in the web version of Teams instead of the desktop client.
- Try uploading the attachment to OneDrive or SharePoint and then sharing the link to the file in the approval request.
