Sharepoint Access App returning 404 errors

I created a Sharepoint Access App in September.  It is a small database, daily updates and bulk import.  Today, these two errors are driving the users crazy - basically the same issue:

1.  check the spelling of the server name.  If it is correct, please contact your system administrator or technical support.  Http Status code: 404

  1. Sorry we couldn't load this app.  We weren't able to load the app's home page.  Please let your administrator know about this. 

Questions are:

1.  why can we not find out where and by who is the server being managed?  I've submitted a ticket to MSIT, escalated to Tier 2 and nothing yet. 

2.  could the dropping and adding of a small table affect the database this much?

    Hi Terry,

    I have checked your SR number and learnt that the engineers there dug into the issue. It seems that you are able to create the Access Web App now. If the issue occurs again, I suggest you contact the support channel where you submitted SR and they will continue to help you further.

    the is is with an app on Sharepoint Online.  ticket is [REG:115112413409149] BUG ID _ 72583 _ MSIT: Access Web App doesn't load on SPO Site
