Automate Failed Flow without reason

Hi Team,

I am having a problem with a "Start an approval process and send an email on Microsoft Form submission" flow. I followed an existing template and the flow worked fine for maybe a week or so then it say failed without giving any specific reason/suggestion.

I then create a new flow similar to this one and it worked well for about a week too then met with the same problem.

This time, I tried to create a new flow but it failed right away.

Would really appreciate if you could help me out on this!!

We would love to help you further, sorry to say that due to limited resource, we cannot fix the issue directly.In this situation, we sincerely recommend you try to create a support ticket via below steps

    1. Power Platform admin center with your admin credentials.
    2. Select Help + support > New support request.

    The support team over there has higher permission than us to investigate the issue from background, they have more resource and correct escalation channel in handling this kind of issues.
