Upgrading my HP Laptpop from Windows 8.1 to Windows 10 via the MediaCreationTool21H1 has failed numerous times for me (no specific error code)
I've already tried troubleshooting the error by:

6 hours and it says it didn't install windows 10. You know I been in computers since the 70s (military computer the size of a house) and started with my Times Sinclair (256 K expansion memory) I am going to stick with my Chromebook because A computer should help you and NOT consume your whole life

I am facing the exact same issue while trying to upgrade my Windows 8.1 to Windows 10.
Steps tried so far :
Tried Upgrading through "MediaCreationTool21H1" - FAILED
Tried Upgrading through "Windows 10 ISO - setup" - FAILED

This should be the first step rather than those guides that tell your to rebuild the MBR, change windows.bt attributes, etc.
I upgraded from Windows 7 so I had to install ISO image mounting software to kick off the upgrade from ISO. I also did not elect to "download updates" while installing.
I experienced a similar problem restoring a Mac via an internet connection.
Restoring from a complete image/ISO is the best solution for these installation failures. The cloud is not so reliable when reinstalling the entire OS. Best to download everything before installation rather than before.

My case was as follows:
I`ve tried to upgrade from Win 8.1 to Windows 10 via Version 21H1 ISO file (run directly from downloaded file as administrator) a couple of
times unsuccessfully. The Win 10 installation process was interrupted each time.
The reason might be an old update files that could no longer be installed, possibly due to an error or expiration.
Finally, have downloaded media creation tool to USB and boot from it.
This time there was the option of the system refreshing.
After refreshing process, upgrade to Win 10 could be run normally without failing.

S. Sengupta MVP.... Not only that U advised him 2 times, about the Clean Install, but that doesn't address the person's need !!! He knew to do that without U saying so, if he wanted to. It is a necessary evil sometimes to keep your files, since U can't reinstall certain applications. ±++++++++++++++++++++++++The way I managed to update from 8.1 to 10 on one of my drives is to recreate the EUFI partition, and do a few no brainier commands, like bootrec /fix boot, fixMbr, rebuildbcd... After that windows 10 installed OK (22H2)
