Profile pictures are not available externally, this is by default, so currently we could not view the picture if it is sent by sharepoint online alert.
The mail sent from alert could not show the profile picture, this is by default.

No, "external" means that two users use different tenant.
Thanks. However, the emails are being read by users of the same tenant that the alert is being sent on. The screenshot I posted is from a user on the same tenant. Shouldn't the picture show? Outlook correctly shows profile photos next to the sender's name, but the SharePoint alerts always show generic gray icons in the email body for users in the same tenant.
Did the user change his profile picture in "My Office Profile"?
No, the profile photo wasn't changed.
All SharePoint alerts from all users show the grey generic people icon. There are no alerts that ever show an actual photo. I see these generic icons too, and I'm a SharePoint admin on the same tenant. Users (including me) show correct profile photos in the Office Profile, SharePoint's Manage User Profiles, Delve, and even the From field in Outlook, but SharePoint alert emails always show the generic icon in the email body.
Which alert are you using? Per my test, I used a flow to send mail from one account to another, I could get the profile picture uploaded by myself:
These are happening on out-of-the-box SharePoint alerts, instead of Flows.
These are the alerts you would get when going to a list/library > click the horizontal ellipsis in the menu bar > Alert Me. Also, this occurs on any list/library and any site collection on our tenant.
