Error while trying to add a "File Upload" question to a Form: "We weren't able to create an upload file folder in OneDrive for Business. If you see this message repeatedly, contact your administrator"

I get this error each time I try to add a "File Upload" question to a Form. As I know it should create a folder "Apps" into OneDrive, and of course, this folder is not created into OneDrive.

I also test with a Group Form (because it is supposed that in this case, it is created automatically a folder into SharePoint), but I get exactly the same error.

    @Lachlan Bunter - 

    This one was frustrating. I never received any explanation from MS as to the issue and the case was closed without my knowledge or approval at the end of September. We are no longer having the problem - so i assume whatever fix they said they initiated in August, that took until September deployment window, was the solution for my tenant. 

    If you haven't, i would open a case with MS directly. 

    Sorry i couldn't offer any other information/help.

    Dear Louie,

      We don't have problem in uploading our own file. 

      While creating TEST for our students, this error occurs in FILE UPLOAD QUESTIONS.

      It is supposed to be created in SHAREPOINT...

    We're having the same issue and have had a case open with support for the past 30 days with no resolution as of yet. Will post back with any info we get.

    Hi everyone,

    I am currently investigating an on-going case with this issue.  It would be great to know if any of the users in your environments experiencing this issue have an apostrophe or accent( ' ) in their username/email/OneDrive URL.  For example: john.O'******@domain.com.  Any feedback would be much appreciated.

    All users in our tenant are having the issue and we do not allow the use of apostrophes in usernames.
