Error while trying to add a "File Upload" question to a Form: "We weren't able to create an upload file folder in OneDrive for Business. If you see this message repeatedly, contact your administrator"

I get this error each time I try to add a "File Upload" question to a Form. As I know it should create a folder "Apps" into OneDrive, and of course, this folder is not created into OneDrive.

I also test with a Group Form (because it is supposed that in this case, it is created automatically a folder into SharePoint), but I get exactly the same error.

    This one was frustrating. I never received any explanation from MS as to the issue and the case was closed without my knowledge or approval at the end of September. We are no longer having the problem - so i assume whatever fix they said they initiated in August, that took until September deployment window, was the solution for my tenant. 

    If you haven't, i would open a case with MS directly. 

    Sorry i couldn't offer any other information/help.

    Case is still open. Received word today that a fix has been applied to SPO but it will be another 3.5 weeks for the rollout period.

    I am a global administrator in our tenant and this issue is still occurring for me and it is now mid December. any Updates on this issue?

    We have the same problems but we had find the cause.

    Our site has the extract/achive option on and this option caused the problem.

    Without that option on all is working but we don't want to disable.

    We are trying to find a solution.

    We just had this issue show up on our side today. We do not have many forms that use this upload feature but one that has been working for a month just broke today. Went to create a test one and it gives the original error above. Going to open a case with MS.
