Branching in Microsoft forms with multiple choice responses

Question

I have Yes, No, and N/A as potential responses/choices on several questions. If response is a No, it branches to a text option to provide feedback; however, regardless of what the response is, I want every question asked (the branched-out text feedback from a No response seems to end the questions in the section and I need every question answered). For instance, if the first three questions are responded with a Yes, and the fourth question is responded with a No, it branches to a text option to fill in the response in freeform; however, the remaining questions in that section do not show up, and it jumps to the questions on the next section. How can I fix this, where every question shows up for a response, regardless of what response is selected?

I went through your description carefully and if I made right understanding of your scenario and demand, below is the way to achieve it so that users will go through all questions and only get Feedback box when they selected No choices.

I just tested this and if a response is a No, the test for feedback pops up (which is intended); HOWEVER, because the test for feedback question is at the end of the line of questions in that section (I have 20 questions in the first section), it jumps to the next section and the questions in that section instead of after inputting feedback, the next question should be the next question in line assuming an answer was Yes or N/A.

Do I have to add separate Feedback text questions for this to work? Your example was Feedback1 and Feedback2. Ideally, I would not want to have to rename each feedback question text box and have the same wording, but I do not think it will work.

I appreciate your patient understanding and sharing with me further.

There has one thing I missed to mention for you yesterday, is that I indeed created each Feedback boxes for each questions, the reason is because if it only contains one main Feedback box in the form, then it won't be able to achieve your demand to make sure users can go over all questions even if they select NO options.

Because I'm really sorry to convey that currently there has a feature limitation of branching in Forms that it doesn't provide the way to branch to a preceding question and will take users directly to the end just like you have experienced before (as mentioned in the thread original description). Hence, indeed I have to convey that my above suggestion is only a workaround to achieve your demand in some way, and if separated Feedback box will lead a more low-efficient experience for you when analyzing the responses, I'm really sorry that currently there has no by-design function to achieve it.

Here is another question.

As the one who created the form and sent it off to some people to fill out, I get the responses; however, is there a way to differentiate the filled-out responses? Here is a snip it of what I am referring to:

I will have no idea who filled it out and for what claim (unless I went in each one and opened it up) it was filled out on and would like to have that option. Same thing for the user/responder, if they fill out multiple surveys on different claims, how can they view it at a glance to see which claim they audited?

Thanks for the attached screenshot for clear reference, I completely understand the demand of easily find out the person who filled the response in Forms portal>Filled forms sections, especially for the form owner to deal with multiple surveys on different claims for a project. But it's really sorry to convey that this is a temporary feature limitation and currently it only displays the forms name and submitted date and time without the responder information until click to open it.

For such a feature limitation, I sincerely recommend you use Feedback Community to suggest this necessary function improvement and add your valuable idea in the****Microsoft Forms · Feedback Community **** which is the best place to share ideas directly with the product building teamand this is the best way to let Forms development team aware of this requirement and how this is indeed required for users dealing with various but similar forms to better management, your valuable ideas and suggestions will help improve the product and make it better for every user.
