My apologies for the delayed response. Most likely reported issue was already resolved. I am providing below list of troubleshooting steps which should help user or user's admin resolve the issues with the MFA Application and failed logins (I suspect most likely account was in the block list):
From AAD Admin side :
- Check if account is not in the block list; force user to Re-register MFA and revoke existing MFA sessions.
From your, end user side :
- Common problems with two-step verification for a work or school account.
- Troubleshoot Azure Multi-Factor Authentication issues.
- Manually add an account to the Microsoft Authenticator app.
