What address to send defective part back to - Microsoft surface pen

I put in a ticket about a surface pen not working. There repair order said to send it back to us within 30 days.

What address do i send this to? is there a packing slip for shipment? How do i send back? What courier? The repair order said no charge, but how am i to send back via mail with no charge?
